usage:

• -e map lut flag missingval
  
• -e static map value flag newvalue
  

LUT:

• 1 column - row number is index, replace with row value
• 2 column - replace values in column 1 with values in column 2

FLAG:

• 0 - keep unmapped values the same (default)
• 1 - set unmapped values to MISSINGVAL
• 2 - set MISSINGVAL to the highest value in LUT
