Buttons passed test
If the I/O Test indicates the buttons work, the problem is likely to be within your paradigm.
Make sure your subject is actually pressing the buttons firmly at the proper times.
If you are running a customized CIGAL paradigm, check that you have included the response box code.
If you are running a ShowPlay paradigm, check that you have not changed the response control variables (resp_flag should be 3, dreadinterval should be > 0).
If you are running paradigms using a program other than CIGAL, run the appropriate testing functions for that software.
